Sciatica is pressure on the Sciatic nerve which is actually two nerves, the tibial and common peroneal nerve, bound together by common sheath of connective tissue. Both of these nerves form the Sciatic nerve, and they originate from L4-S3. Pressure or injury to the sciatic nerve results in sciatica. Many cases of sciatica can be helped by chiropractic services. When a person has sciatica, the first step is identifying the cause of the sciatic pain pattern. Orthopedic exams, neurological testing, and other diagnostic testing such as MRI's can help determine if the Sciatica is cause by herniated disks, or other reasons. Many times, Sciatica is caused by what has been coined Piriformis Syndrome. The Piriformis muscle is a small triangular muscle deep in the buttocks. The Sciatic nerve passes either through the Piriformis muscle, or underneath it depending on how a person is born. When the Piriformis muscle is irritated, it can contract in to a chronic state of spasm. This often times will press on the Sciatic nerve.

Through chiropractic services, we can diagnose the cause of the sciatic pain, and properly correct the Sciatica. If the Sciatic pain is caused by Piriformis syndrome, certain chiropractic treatments are utilized to relax the Piriformis muscle and lessen the nerve root irritation. In my experience as a NYC chiropractor since 1998, I've had many cases of chronic sciatica heal over a relatively short period of time once the piriformis muscle spasm had been addressed and corrected.

If the Piriformis muscle is not involved, and the cause is L4-S3, it is possible that a malposition of the vertebrae in the lower spine is causing impingement and nerve root irritation on the nerves that form the Sciatic Nerve. We call this a subluxation. A subluxation is the malposition of a vertebrae causing the nerve to be irritated. After determining through a chiropractic exam, and using diagnostics such as x-rays, we can find the possible subluxation that is contributing to the Sciatica, and restore proper alignment and biomechanics to that vertebrae and the adjacent one causing the irritation. Freeing up the subluxation many times helps the sciatica in relieving the pressure on the nerve, and healing the pain. Finally, a herniated disk may be involved in the sciatic pain. Incorporating chiropractic, physical therapy, and disk decompression is often very effective in restoring integrity to the disk, and allowing the body to heal.

Chiropractic Treatment of Sciatica

Sciatic nerve pain varies in intensity and frequency; minimal, moderate, severe and occasional, intermittent, frequent or constant.

The pain is described as dull, achy, sharp, toothache-like, pins and needles or similar to electric shocks. Other symptoms associated with sciatica include burning, numbness and tingling sensations. Sciatica is also called radiating or referred pain, neuropathy, or neuralgia. A misconception is that sciatica is a disorder - however, sciatica is really a symptom of a disorder.
A Sciatica is generally caused by sciatic nerve compression. Disorders known to cause sciatic nerve pain include lumbar spine subluxations (misaligned vertebral body/ies), herniated or bulging discs (slipped discs), pregnancy and childbirth, tumors, and non-spinal disorders such as diabetes, constipation, or sitting on one's back pocket wallet. A common cause of sciatica is Piriformis Syndrome. Piriformis syndrome is named after the piriformis muscle. The piriformis muscle is located in the lower part of the spine, connects to the thighbone, and assists in hip rotation. The sciatic nerve runs beneath the piriformis muscle. This muscle is susceptible to injury from a slip and fall, hip arthritis, or a difference in leg length. Such situations can cause cramping and spasm to develop in the piriformis muscle, thereby pinching the sciatic nerve and causing inflammation and pain.

Sciatic nerve compression may result in the loss of feeling (sensory loss), paralysis of a single limb or group of muscles (monoplegia), and insomnia. Since there are many disorders that cause sciatica, the Santa Barbara Chiropractic first step is to determine what is causing the patient's sciatica. Forming a diagnosis involves a thoughtful review the patient's medical history, and a physical and neurological examination. Diagnostic testing includes an x-ray, MRI, CT scan and/or electrodiagnostic tests (nerve conduction velocity, electromyography). These examinations and tests help to detect possible contraindications to spinal adjustments and other chiropractic therapies.

Sciatica can be caused by other disorders beyond the scope of chiropractic practice. If the Doctor of Chiropractic determines the patient's disorder requires treatment by another type of doctor, then the patient is referred to another specialty. In some cases, the referring chiropractor may continue to treat the patient and co-manage the patient's care with the specialist.

The purpose of chiropractic treatment is to help the body's potential to heal itself. It is based on the scientific principle that restricted spinal movement leads to pain and reduced function and performance. Chiropractic care is non-invasive (non-surgical) and drug-free. The type of chiropractic therapy provided depends on the cause of the patient's sciatica. A sciatica treatment plan may include several different treatments such as ice/cold therapies, ultrasound, TENS, and spinal adjustments - sometimes called manipulation.

1. Ice/Cold therapy reduces inflammation and helps to control sciatic pain.
2. Ultrasound is gentle heat created by sound waves that penetrates deep into tissues. Ultrasound increases circulation and helps to reduce muscle spasms, cramping, swelling, stiffness, and pain.
3. TENS unit (transcutaneous electrical nerve stimulation) is a small box-like, battery-powered, portable muscle stimulating machine. Variable intensities of electrical current control acute pain and reduce muscle spasms. Larger versions of the home-use TENS units are used by chiropractors, physical therapists and other rehab professionals.
4. Adjustments (Spinal Manipulation) At the core of chiropractic care are spinal adjustments. Manipulation frees restricted movement of the spine and helps to restore misaligned vertebral bodies to their proper position in the spinal column. Spinal adjustment helps to reduce nerve irritability responsible for causing inflammation, muscle spasm, pain, and other symptoms related to sciatica. Adjustments should not be painful. Spinal manipulation is proven to be safe and effective.

In college and during their training, students of chiropractic learn many different adjustment techniques enabling them to treat several types of subluxations and disorders. Techniques vary from a swift high velocity thrust to those that combine minimal force and gentle pressure. Mastery of each technique is an art that requires great skill and precision. Chiropractic Services vs Spinal Surgery

Chiropractic services is gaining popularity as a treatment for scoliotic patients since it has been proven to be just as effective, if not more, than the existing treatment options for scoliosis. Chiropractic does not only treat any existing problems but also prevents further progression of the curves that is causing the problem. Those patients who’ve undergone chiropractic care for scoliosis have shown momentous improvement in size of the curves and this include [sic] 70 percent of them. This is important since any relief from scoliosis means that a patient won’t have to undergo surgery, take medications, and suffer from the effects of scoliosis.

That is not to say, however, that conventional treatment is beyond criticism.  Negrini (2008) argues that, in treating AIS, “[...] in clinics exercises are generally ignored; braces are used with some criticism, while fusion is generally considered the only reliable treatment.”  And this despite evidence of efficacy for exercises and bracing being, in her opinion, better than that for surgical fusion (“grade B, B and C recommendations, respectively”)  She continues, making the point that, “The interest of the AIS treatment community (composed almost exclusively by orthopedic surgeons) has shifted toward fusion […] while conservative treatment is suffering a decrease in professional interest.”

The views of this author could be coloured by a commitment to exercise as a treatment, however, it is an interesting view backed up by evidence – in contrast to the propaganda offered by this pro-chiropractic services.  “No controlled study, neither short, mid nor long-term, was found to reveal any substantial evidence to support surgery as a treatment for this condition. There is some evidence supporting the conservative treatment for AIS. No substantial evidence has been found in terms of prospective controlled studies to support surgical intervention. In light of the unknown long-term effects of surgery, a randomised controlled trial (RCT) seems necessary. Due to the presence of evidence to support conservative treatments, a plan to compose a RCT for conservative treatment options seems unethical. But it is also important to conclude that the evidence for conservative treatments is weak in number and length.”
Given this, it is odd that UK’s Scoliosis Association are very positive about appropriate surgery; as are the UK’s NHS and support4scoliosis.  This apparent dichotomy may indicate that the evidence base for conventional treatments could do with improving.  However, it does not provide a justification for the use of chiropractic services.  Having looked at what evidence I can find, it’s clear that it isn’t decisive for any conservative intervention.  However, it seems to be pretty reasonable to think that bracing (Rowe, et al, 1997) and exercise have merit. It’s also clear that surgery is widely recommended by credible authorities, although the evidence base would seem to be sparse.  This makes me wonder: is this because it’s not as effective as people think, or is it because RCTs are hard to justify ethically and conduct for surgical interventions?  I don’t know and would be interested to hear from people who do.

So, what of the ‘package’ of potential treatments listed in this article?  One thing is very clear: there is absolutely no good reason to suppose that chiropractic services spinal manipulation offers anything beyond making some people feel better if they are in pain. Shoe lifts might have some merit, but they have been insufficiently researched for anyone to say. Electrical stimulation is a treatment whose time has passed.  For a while it looked like it might be effective, now the best available evidence says otherwise.

The most truthful claim in the article is the implication that, “isotonic or active exercise methods” might be useful.  I am not sure that these specific exercises have much merit, but there are examples of exercise regimes in the literature that look like they do.  At the end of the day, AIS is a condition that mostly resolves itself over time.  For the 10% of children who need treatment, braces and exercise look like the only conservative options.  For those whose condition is the most severe, the decision to try the surgical option must be a difficult one.

The combination of a large number of cases that will get better anyway and limited –as well as intimidating – conventional options provides the classic environment for marketing CAM ‘treatments’.  The kind of opportunistic plugging of un-evidenced and disproven therapies seen in this article can only serve the financial interests of those who make a living from selling these treatments: it cannot serve the interests of patients and their families.  Neither does it help the broader challenge of bringing cost-effective medical care to communities as a whole.

Unless and until advocates for alternative therapies can provide decent evidence for the interventions they espouse, or at least discuss the evidence in a truthful and balanced way, I would really like to see them showing some modesty and limiting their claims to helping people to feel better; as opposed to actually making people better.

Possible Chiropractic Services Approaches for Sciatica

Pero's team measured 107 individuals who had received long-term Chiropractic care. Proper chiropractic care during pregnancy is an important, yet often overlooked, aspect of prenatal care. The rapid growth of the uterus during pregnancy results in stress on the pelvis and spine as a woman’s center of gravity changes. As a result, many pregnant women experience back pain, pelvic pain, and sciatica due to the compromised musculoskeletal system and nervous system. Additionally, when the pelvis and spine become misaligned due to the stresses placed on them the position of the baby is less than optimal. Chiropractic adjustments restore alignment and proper nerve function resulting in decreased pain and better position of the baby which in turn can result in easier, faster, and safer births.

The uterus is supported in the pelvis by three sets of ligaments: the broad ligament, the round ligaments, and the uterosacral ligaments. These ligaments are attached to the bones of the pelvis. When the pelvis is unbalanced or misaligned (or subluxated as chiropractors call it) the ligaments will become unbalanced as well. As a result these ligaments pull unevenly on the uterus. This torsion of the uterus results in intrauterine constraint for the baby which can result in breech presentations (baby is head up instead of head down), transverse presentations (baby is sideways), or extension and/or rotation of the baby’s neck. Any of these deviations from the ideal head-down, chin-tucked position will increase the likelihood of a longer and more difficult labor as well as increased interventions in labor such as epidurals, episiotomies, forceps, and Cesarean sections.

Subluxations of the low back and pelvis also affect the functioning of the nerves that supply the uterus. During labor these nerves cause dilation of the cervix and the contractions of the uterine muscles. If the nerves are not functioning at their most optimal level due to a subluxation then labor may be longer or more difficult.

Chiropractors who treat pregnant patients often use the Webster technique to balance the pelvis and influence the nerves of the low back and pelvis. The Webster technique is a very specific chiropractic adjustment that reduces interference to the nervous system and promotes balance in the bones, muscles, and ligaments of the pelvis. When using the Webster technique the chiropractor will check for the presence of a sacral subluxation (misalignment of the sacrum). Medicare Part B pays for a chiropractor’s manual manipulation of the spine to correct a subluxation (when one or more of the bones of your spine move out of position). Additionally, manual devices (i.e., those that are hand-held with the device being controlled by hand) may be used by chiropractors in performing manipulation of the spine. However, no additional payment is allowed for the use of the device.

Medicare does not cover the following services performed by Santa Barbara chiropractors:

Initial physical examinations, Evaluation services, X-Rays, Acupuncture, Physical therapy, Vitamin, mineral and/or food supplements, or other supplies and Orthopedic devices.

Your chiropractic treatment is covered by Medicare if:

You have a significant health problem in the form of a neuromusculoskeletal condition that needs treatment (such as pain, inflammation, swelling, leg and foot numbness, etc.)
You have a subluxation of the spine.
Manual manipulation has been recommended for improvement of your condition.

What Do I Pay?

You will have to pay the Part B deductible, and then 20% of the Medicare-approved amount. Medicare reimbursement will be limited to one visit per day, unless there is a medical need for more than one treatment each day.


The hands-on nature of the chiropractic treatment frequently requires patients to visit the chiropractor a number of times. To be treated by a chiropractor, a patient needs to go into the office.   • Once the maximum therapeutic benefit has been achieved, continuing chiropractic care is considered not medically necessary.
  • Chiropractic manipulation in asymptomatic persons or in persons without an identifiable clinical condition is considered not medically necessary.
  • Chiropractic care in persons, whose condition is neither regressing nor improving, is considered not medically necessary.


All states require that a chiropractor be a graduate of an accredited chiropractic college.
